The REACH FAR Keep on Track (KOT) Intervention was implemented in four Asian American faith-based communities in metropolitan New York City (NCY) and New Jersey. KOT is a program developed by the NYC Department of Health and Mental Hygiene. The program included regularly scheduled screening and counseling events with congregants, which were volunteer-run and community-based. Outcomes included reductions in blood pressure and increased health-related self-efficacy. Health-related self-efficacy significantly improved after six months, and reductions in blood pressure were seen among participants reporting a previous diagnosis of hypertension.
