DREAM Prediabetes Protocol Paper

This paper is a study protocol for the DREAM Initiative, a randomized controlled trial testing an intervention that combines electronic health record (EHR) tools and community health worker (CHW) support to promote weight loss among South Asian patients at risk for diabetes. The planned sample consists of South Asian adults with elevated diabetes risk, recruited in a clinical context where both community-based and health system supports could be integrated.
