The Health + Housing Project was a CHW program located in two subsidized housing buildings in New York City (NYC), which aimed to address the residents’ self-identified health-related needs, including social and economic risk factors. The goal of the project was to expand CHW models by designating subsidized housing as the recruitment and intervention site, inviting all adult residents to participate. Working with a CHW, residents set health-related and life-improvement goals and developed an action plan to help achieve them. At the end of the program, residents completed a post-survey to assess change in overall wellness, connection to community resources, program satisfaction, their experience working with the CHW, and whether they felt that they had achieved their primary goal.
