SOCB REDCap xml file for upload which is used to track the patient navigation process by the CHWs. It includes: 1) An intake form which is completed by the CHW upon their initial interaction with a patient to capture intake information, such as referral date, type of patient, and initial needs. 2) A patient encounter form which is used to track each interaction with a patient, including what needs were addressed during the encounter and how they were addressed. 3) A needs status form which is used to provide an overview of identified needs of the identified needs of the patient stand in terms of completion. 4) An appointment tracking form which helps CHWs keep track of appointments made and whether or not they were attended by the patient.
