IMPACT EHR Outcomes Paper

This study examined the feasibility and outcomes of an electronic health record (EHR)-based intervention to improve hypertension management in primary care practices serving immigrant communities. The intervention focused on using EHR tools within routine care to support better identification, monitoring, and management of patients with high blood pressure.
