HTN Initiative Baseline Patient Survey for ADHERE

The NYU Hypertension Initiative aims to improve blood pressure control for NYU Langone Health patients. This program is aligned with the projects ADDRESS-BP (Actions to Decrease Disparities in Risk and Engage in Shared Support for Blood Pressure Control in Blacks) and ADHERE (Addressing Disparities in Hypertension and Reducing Racial Inequities through Engagement).
