HHAP Publication 2024

This publication describes Harlem Health Advocacy Partners (HHAP), a place-based community health worker (CHW) program led by a local health department to address chronic disease and social needs in Harlem. The program focused on residents in a defined neighborhood context—particularly those facing high burdens of illness and structural barriers to care—and illustrates how a public health agency implemented CHW services at the community level.
