This report summarizes the community needs assessment conducted to inform the launch of the Harlem Health Advocacy Partners (HHAP) initiative, a neighborhood-based community health worker program. The assessment focused on understanding the health needs, barriers to care, and social challenges affecting Harlem residents—particularly those living in public housing—to guide the design of a community-responsive intervention.
