DREAM Diabetes Protocol Paper

This publication is a study protocol for the DREAM initiative, a multi-level community–clinical linkage intervention designed to improve glycemic control among South Asian patients with uncontrolled diabetes. The planned sample consists of South Asian adults with poorly controlled diabetes, and the intervention links community-based support with clinical care to address barriers to diabetes management.
