This publication reports the protocol and baseline characteristics for the DREAM Atlanta study, a community health worker–led program designed to improve hypertension and diabetes management among South Asians in Atlanta. The sample consisted of South Asian adults in the Atlanta area with hypertension, diabetes, or both, and the article describes both the planned intervention and the characteristics of participants enrolled at baseline.
