This study evaluated a culturally tailored community health worker (CHW) intervention for immigrant patients with type 2 diabetes and found improvements in patient-centered outcomes. The intervention was designed to support diabetes self-management in a way that reflected participants’ cultural and community contexts, through education, navigation, and ongoing support.
